  • The following form is only for Transfer Students who completed a Practicum Course at another institution.  If you successfully completed a practicum course at another institution, or a course that transferred in as CMU's EDU 107 (Introduction to Education) equivalency that has a fieldwork component as part of the course, you may submit the following form to receive credit for the field/observation hours you spent in a PK-12 classroom as part of that course. You may only claim the number of hours actually spent in a classroom with the PK-12 students on this form. Any additional observation hours you completed on your own must be submitted on the "Field Experience/Observation Hours Form" above.  NOTE: this is not for students who completed the EDU 107 course at CMU since you are automatically credited for the 10 hours (15 hours for EDU 107H) associated with the course.

  • If the school in which you observed does not meet the Race/Ethnicity category, but the classroom itself did at the time you observed, you may submit the "Classroom Race/Ethnicity Documentation Form" below for documentation.  This form is ONLY submitted as an addendum to a "Field Experience/Observation Hours Form" you are submitting or have submitted for this same Experience/Observation. 
    • (EXCEPTION:  If you need to document that the individual classroom in which you were placed for your Pre-Student or Student Teaching experience met the Race/Ethnicity category even if the school did not, you may submit the following form by itself.)                   
NOTE: Check the Diversity Table prior to filling out the form below.  If the school already fulfills the Race/Ethnicity category, do not submit this form. You will automatically receive credit when your Field Hours are entered.
