All students admitted to the M.S. in Nutrition and Dietetics should complete and submit the following two forms, upon admission:

1. Graduate Student Planning Form (The planning form should be completed after a student has been grantedadmission to the M.S. in Nutrition and Dietetics program. In advance of completing the planning form, please review the Informational Handout to Complete the Planning Form.)

2. The Demographics Form, accessible at:

All students should complete and submit the following survey near the end of their final semester in the program:

Credit By Examination (CBE) Application Form is available by clicking on CBE Form.

Students who are seeking a faculty sponsor for their Plan B paper or thesis should complete the application for thesis sponsor.
