Some of the services we provide require you to have a referral from your family or primary care physician in order for us to bill your insurance company. We will be asking you questions when we schedule your appointment to help determine if a referral is needed.

If a referral is needed, inform your doctor's office when you are scheduled for your next appointment at the Carls Center.  Also, give your doctor enough lead time to facilitate any prior authorization steps and to issue a referral form or prescription of services.  Your doctor may use the Carls Center referral form or simply write a prescription detailing pertinent diagnoses and functional needs. If you are having difficulty getting the needed referral from your doctor, please let us know as soon as possible before your scheduled appointment date, so we can provide assistance and help to avoid any delays in our seeing you.
