Submission Deadline: On or before the first class meeting as enrolled in ATR 280 . Packets will not be accepted past the deadline. Students who have submitted late, incomplete, or improper packets will be administratively dropped from ATR 280 for that semester. A selection opportunity will not be counted against the student and the student will need to apply in a subsequent semester.

Required Application Materials:

• Application form
• Copy of immunization records
• Verification of Practical Experience Form.
• The form(s) must verify 120 hours of practical experience related to athletic training.
• The form must be secured in a sealed envelope with supervisor signature across the envelope.
