The SLP Application for Admission and Recommendation forms are Word documents created in "form" format.  The SLP Major Grade Workseet is an Excel spreadsheet and MUST be save to and completed on your computer, as it will automatically calculate your "major" GPA.  Only inculde those courses that were required for your major.
