Nomination Forms

This award is presented to a graduate in recognition of his/her distinctive service of a humanitarian nature orcitizenship through community and public service activities.

Criteria:
  1. Must be an active member of a local civic or community group and lead or participate in ongoing activitieson an annual basis. (Worth up to 50 points)
  2. Received honors, awards, or public recognition for contributions to local or community groups. (Worth up to 30 points)
  3. Provided volunteer service to the University. (Worth up to 20 points)
