Nomination Forms

Criteria:
The candidate for this award will be a graduate or friend of CMU. They will have demonstrated outstanding volunteer service to CMU. The candidate must possess the highest standards of integrity and character to positively reflect and enhance the prestige of CMU. The candidate must have demonstrated continuing outstanding voluntary service to CMU by working with alumni clubs or alumni chapters.
