Once you have selected a major, you will need to request a faculty advisor from the School of Health Sciences. Please click link below, undergraduate advisor request form or stop in the main office, Health Professions Building, room 2219, to fill out an advisor request form. Upon completion of this form an advisor will be assigned to you. You will be given your adviser's office hours and contact information. You will be responsible for getting in touch with your assigned advisor.

*Students planning to major in health fitness. Effective Spring 2009 you are required to complete an application for acceptance into this program. Go to the following website for an application.
