How do I schedule an appointment?

You may directly call 989-774-3904 to  schedule an appointment with the Carls Center.

Alternatively, you may have your health care provider, family member, caretaker or guardian call 989-774-3904 on your behalf to schedule the appointment. 

Please be advised, we do ask that the person making the appointment on your behalf has access to information regarding your health insurance (including carrier name and contract numbers) and your family or primary healthcare physician.)

Rescheduling

Please be courteous and help us with accommodating our clients. 

If you are unable to keep your appointment, please call 989-774-3904 as early as you realize that your appointment time cannot be kept.  This way we can offer your open timeslot to another client who may be on our waiting list. 

Inclement Weather or Community Emergency

In case of inclement weather or community emergency, please call the CMU Information Line at (989) 774-7500.  If CMU is closed, the Carls Center will also be closed.  We will call to reschedule your appointment during the next business day.
