BEFORE YOU CLICK ON THE "ACCEPT AND AGREE" BUTTON AT THE END OF THIS AGREEMENT, CAREFULLY READ THE FOLLOWING:

Though precautions are taken for all activities and opportunities associated with Leadership is Central, there is no guarantee of absolute safety against injuries or unforeseeable accidents. Reading and agreeing to the terms below serves as a release of liability for Central Michigan University, the Leadership Institute, and all volunteers and employees associated with Leadership is Central.

I acknowledge the program itinerary and conditions set forth and agree with the contents in their entirety. I recognize that my child may participate in activities such as (but not limited to) community service projects, rock climbing, cardboard boat races, activities in the student activity center, and/or other physical activities. All of these activities require an element of physical exertion and/or risk. Though activities may not be strenuous, I understand that injuries, unforeseeable events, or medical complications may occur. I assume full responsibility for personal injuries, accidents, or illnesses. I also assume responsibility for damage to or loss of personal property. I also acknowledge that my child may decline to participate in any activity though he/she may be healthily encouraged to do so.

I understand that staff members will make attempts to communicate with me prior to any medical treatment in non-life threatening and other non-emergency situations. In life-threatening or emergency situations, medical examinations and treatments may be performed without necessarily communicating with me first. I give consent for my child’s transportation to a medical facility and for the release of any records necessary for insurance purposes. I also agree to pay all costs of rescue and/or medical services that may be incurred by my child.

I recognize that Leadership is Central staff members have the right to terminate the participation of any person violating camp rules, suffering from severe cases of homesickness, or any other grounds for discharge. In the case of my son/daughter’s removal from camp, I assume personal responsibility for the unplanned travel to CMU to remove my child from university premises. I also understand that early removal from camp will not result in any type of refund or reimbursement.

I authorize Central Michigan University, the Leadership Institute, and all other individuals and organizations associated with Leadership is Central to take and use any photographs, statements, and/or videos of my child for camp and university publicity purposes.

This document serves as written permission for my child to participate in all activities associated with Leadership is Central. I have read and understand the above parent release form. I hereby authorize Leadership is Central staff and volunteers for the care of my child.

CAMP RULES & GUIDELINES
  1. Everyone must attend all scheduled events.
  2. Students are to respect all facilitators, administrators, and chaperones and follow their instructions. Students are also to respect fellow campers and the CMU campus including residence halls, buildings, property, etc. Athletes and their family will be held financially responsible for any damages incurred. Loud noise and other intrusive behaviors are prohibited.
  3. Students will be roomed according to gender. At no time will opposite genders be allowed in each other’s rooms.
  4. Firearms, knives (or any other weapons), fireworks, alcoholic beverages, tobacco products, and other drugs (unless prescribed by a physician) will not be tolerated.
  5. Students MUST be in their designated rooms by curfew.
  6. Participants will be required to wear name badges at all times.
  7. Students will stay in groups/pairs at all times. Do not travel alone or leave the group.
  8. Dress for the camp should be socially acceptable.
  9. All valuables are the sole responsibility of the camper. These should be kept properly safeguarded. Central Michigan University, the Leadership Institute, and all other affiliates are not responsible for stolen and/or damaged property.
  10. The use of cellular phones, pagers, personal computers, and other electronic devices are prohibited during all sessions. We do, however, recognize that students likely will have these on campus so they may be used during free time, breaks, and evening activities.
  11. If participants are driving themselves, all keys must be turned in to the camp coordinator. Personal vehicles may not be used during camp.
I have read, understand, and agree to abide by the above rules. If at any time I choose to violate these rules, I understand that I may be discharged from the program without refund or any type of reimbursement.
