Complete one requestor information form per class being taught.
Information needed on the Requestor Information Form is your
name, address, phone number, e-mail address, course number and
title, and the beginning and
ending dates of the class. This form also lets you detail your
requirements for copying your course pack (e.g., stapling,
shrink-wrapping, etc.).
Please note that it is standard operating procedure to put the
course pack in order alphabetically by authors’ last names. If
you prefer some other order, please specify.
