The first step in ordering a coursepack is completing the Requestor Information Form.
Please complete this form (one form per class being taught) to provide
your personal and course information before submitting the specific
copyright permission request forms below.
Use the forms below to request copyright permission for
coursepack materials. A form must be submitted for each item you want to
include in the coursepack.
NOTE: If you are re-using a coursepack from a previous
semester, you only need to complete and submit the Requestor Information
Form for that class. You do not need to submit the permission forms
again.
