To Fill a Prescription

New Pharmacy Users

The first time you bring in a prescription, we will ask you to provide certain information about yourself for our records.  Be sure to have your prescription insurance card or photocopy of it (front and back) with you. We will also have you sign a form acknowledging that we have offered you our Notice of Privacy Practices as required by the HIPAA Privacy Rule.

New Prescriptions

New prescriptions can be dropped off at the pharmacy (Foust 256) or called (989-774-6590) or faxed (989-774-6665) directly from the physician's office to the pharmacy.

To Transfer A Prescription

Bring your bottle, label, or prescription to the pharmacy or call ahead (989-774-6590) with the name and phone number of the Pharmacy where the prescription was last filled.

The CMU Pharmacy staff will contact the outside pharmacy and arrange the transfer of the prescription. Please allow 1-2 business days for the request to be processed.
