​​​​What should I expect when I arrive for my appointment?

Arrival ​Time:Your appointment time is reserved just for you. We ask that you arrive 10 minutes before your scheduled appointment to complete a short registration process.
Late Arrival:Our offices make every effort to stay on schedule. If you do not arrive on or before your scheduled appointment time, it may be necessary to reschedule your appointment.
Cancellations:If you cannot keep your appointment, please call our office immediately to cancel. If at all possible, please call us at least 24 hours prior to your scheduled appointment.

What should I bring to my appointment?

Please bring all your current medications with you to each appointment. (See medication review​)

As a patient, you are responsible for providing current insurance information (health insurance card or a photocopy of it) at the time of service.

Co-pays and co-insurances are the responsibility of the patient and are expected at the time of service.
