Placement Forms

Placement Forms
The Pre-Student Teaching Placement Application is used to gather necessary information so that you can be placed in your Pre-Student Teaching Field Experience.  It is important that when filling out the schedule you include the courses for which you are registered as well as any other scheduling conflict (work schedule, etc.).

  1. Treat this as if it were a job application.  Use complete sentences, proper grammar, and correct spelling.
  2. Proofread your work, and have someone else proofread it as well.
  3. This is your chance to make a good first impression with your host teacher.  Give your host teacher a reason to be excited to have you in his/her classroom.
  4. Be descriptive.  Give the host teacher a chance to get to know who you are before you enter the classroom.
  5. This form gives your permission to use your likeness, voice and words in television, radio, film or in any form to promote activities of Central Michigan University and/or Mount Pleasant Public Schools.
  6. Once the form is completed and submitted there is not an opportunity to make changes.

PLEASE NOTE:

Every attempt will be made to accommodate your schedule and individual needs.  However, if your placement application is submitted with errors, you may lose priority with placements and you will be asked to re-apply.  If the second attempt contains errors, you will risk not be placed and may be asked to postpone your Pre-Student Teaching Field Experience until the following semester.
