An Incident Report is used to report information/observations to other university officials at CMU. You never know who will read your incident report, therefore the Office of Student Conduct has provided the following video presentations to assist you with submitting your incident report.

The Basics​ - This video will show you the basics of how to write an incident report including what your first two sentences should be. Make sure you follow the template for all reports.

The Advanced Class - In this video we look at what content should be included in well-written incident reports. 

Using Advocate - Incident Reports at CMU should be entered into the Advocate system at Central Michigan University. This video will show you how to fill out a report once you have logged into the Advocate system.
