PD 10/20/30 Plan
The PD 10/20/30 Plan is a mandatory generic plan with a 10% co-pay for generic prescriptions. This plan automatically fills with a generic prescription if a generic is available. There is a 20% co-pay for brand drugs on the
formulary/drug list that do not have a generic equivalent. There is a 30% co-pay if there is no generic available and the brand drug is not on the
formulary/drug list. If a brand drug is used and the brand drug has a generic equivalent, the cost to the employee will be the difference between the brand cost and the generic cost plus the applicable brand co-pay (20% or 30%) of the full cost of the brand drug. If an out-of-network pharmacy is used, there is a 50% co-pay.
PD 20/50 Plan (Closed to participants not enrolled in plan as of 7/1/2012)
The PD 20/50 Plan is a dispense as written (DAW) plan with a 20% co-pay for generic drugs and a 20% co-pay for brand name drugs on the
formulary/drug list, when a generic is not available. There is a 30% co-pay if neither the brand nor the generic drug is on the
formulary/drug list. If an out-of-network pharmacy is used, there is a 50% co-pay.
