Q. Must I use the EyeMed network or providers to participate in this vision plan?

A. The EyeMed Vision Care plan allows for both in-network and out-of-network use. However, the greatest cost savings for you comes from using the in-network. Please refer to the Summary of benefits table for a comparison between in-network and out-of-network benefits and costs provider. Before deciding to enroll in this plan, you should review the list of participating providers in the National Directory (located in the Benefits office) to determine if the provider network suits your needs. You may also call EyeMed Member Services toll-free at 1-877-226-1115 or visit to locate the nearest EyeMed provider.
You can mix and match in-network and out-of-network providers/services. For example, you could decide to use an out-of- network doctor for your eye exam and then bring your prescription for lenses to be filed at an in-network eye care center.
Please be aware that, if you are a contact lens wearer, there are some professional services a provider must conduct in order to dispense your lenses. Please discuss these services and any additional fees you could be responsible for before choosing to obtain contact lens materials from another provider.

Q. What if my current physician does not participate in the EyeMed network?

A. Your physician may contact EyeMed about joining the network or we can ask EyeMed to make the contact. However, we are not able to assure you that your physician will participate, so consequently you should not base your enrollment decision on the assumption that your physician will join the network.

Q. What advantages are there in signing up for the EyeMed plan?

A. In addition to cost savings associated with eye exams, and purchasing of eyewear (lenses and frames), as a plan participant you will also be eligible for a 15% discount on Lasik or PRK vision correction through the U.S. Laser Network. This network is administered by LCA-Vision.

Q. How will I know where I can go to take advantage of the discount on LASIK or PRK vision correction?

A. You will need to check the directory of U.S. Laser Network providers. LCA-Vision, the administrator of this network, operates 22 centers and manages a national network of laser vision correction providers. You may call the U.S. Laser Network's toll-free customer service number at 1-877-5LASER6.

Q. Can I buy prescription sunglasses instead of regular classes with my allowance?

A. Yes.

Q. Will my EyeMed plan cover frames separately from lenses?

A. Yes. In addition to frames, your vision benefit will also cover contact lenses OR frame lenses in one plan year.

Q. After I have exhausted my vision benefit for the plan year, do I get any additional discounts?

A. Yes. Even though you may use all of your vision benefit in a plan year, you will still be eligible to receive from 20% to 40% discounts on eyewear.
