Steps for Employees & Supervisors to Follow When Preparing for a Leave of Absence

Employee Responsibilities:

  • Determine what type of leave applies to your situation.
  • Find out how much sick/vacation time you have available to help you determine if leave will be paid or unpaid.
  • Notify your supervisor of your need for a leave of absence as soon as possible.
  • For Staff employees, contact HR/Employment Services (Staff) at 989-774-2010 to determine what forms must be completed, if needed. For Faculty employees, contact Faculty Personnel Services (Faculty) at 989-774-3368. Contact HR/Benefits & Wellness at 989-774-3661 to understand what happens to your benefits while on your leave of absence and to obtain short-term or long-term disability paperwork, if appropriate.
  • Submit appropriate paperwork to your physician for completion, if required. Physician must return completed paperwork to you.
  • Complete appropriate paperwork requiring your personal information, if needed.
  • Submit all completed paperwork to your supervisor.

Supervisor Responsibilities:

  • Once employee notifies you of their need for a leave of absence, determine what type of leave applies to your employee’s situation.
  • Find out how much sick/vacation time your employee has available to help determine if leave will be paid or unpaid.
  • For Staff, contact HR/Employment Services (Staff) at 989-774-2010 to determine what forms must be completed, if needed. For Faculty, contact Faculty Personnel Services (Faculty) at 989-774-3368.
  • Notify your employee to contact HR/Benefits & Wellness at 989-774-3661 to understand what happens to their benefits while on their leave of absence and to obtain short-term or long-term disability paperwork, if appropriate.
  • Provide employee with appropriate forms requiring information to be completed by their physician, if required. Notify employee to return all completed paperwork to you.
  • Forward all completed paperwork to HR/Employment Services at Rowe 109 (staff) or Faculty Personnel Services at Warriner 308 (faculty).
  • You must verbally notify your employee that their leave of absence qualifies or doesn’t qualify as FML within 2 days from date you were notified of the upcoming leave.
  • You must provide written notification to your employee that their leave of absence qualifies or doesn’t qualify as FML before the beginning of the next pay period after you were notified of the upcoming leave.
  • Forward a copy of the written notification to HR/Employment Services at Rowe 109 (staff) or Faculty Personnel Services at Warriner 308 (faculty).
