Before proceeding, you must gather the following documents. You will submit them electronically in the next step, so be sure to scan them or take a picture with your phone.

Biographical page of your passport - make sure all information is visible, including the readable strip at the bottom of the page.

U.S. visa - if you changed status to F-1, you may use your I-797 approval notice instead; if you are a resident of Canada, you may use your I-94 instead.

Most Recent U.S. Entry Stamp - this is often located near the page where your visa is located (SEE EXAMPLE). Only the most recent entry stamp is needed. Travellers entering the U.S. from a land port may have this stamp on a piece of paper stapled in the passport.

Most Recent I-94 - this can be downloaded from the CBP I-94 site (DOWNLOAD NOW); print or save as a PDF for uploading in the next step.

I-94 Travel History - this can be downloaded on the same page as the I-94; simply click on the "Get Travel History" button after saving your I-94. Print or save this as a PDF for uploading in the next step.
In addition to these items, you will also need to know your U.S. physical address (where you are/will be living) and your U.S. phone number (if you have one).
