Mobile Health Central Event Request Form

Mobile Health Central (MHC) operates mobile clinics in underserved areas in partnership with community organizations, schools, businesses, and other groups dedicated to health improvement.  Past requests have consisted of grant funded health screenings, audiology/hearing testing, sports physicals, wellness screenings, and immunization clinics.  Senior centers, local government facilities, K-12 schools, county fairs, and community centers are some of the more popular entities providing services.

To request MHC, please complete the event request form to us at least three months prior to your event. Upon receipt of your request, we will contact you within ten business days to discuss your event and let you know if Mobile Health Central is available. We cannot guarantee availability for all requests.
