File A Report

A report may be submitted to the Office of Student Conduct by any member of the University Community.  To file a report, click on the link below.  You will be asked to enter information pertaining to the incident you are reporting.  You may file an anonymous report, however, without providing your name and contact information, it may be very difficult to investigate the incident you are reporting.

File an Incident Report

Anyone may use this form to report suspected violations of The Code of Student Rights, Responsibilities, and Disciplinary Procedures (Code of Conduct). Examples of reportable incidents include classroom disruptions, alcohol or drug-related misconduct, fights, threats, hazing, and other potential violations of the Code.

Report an Academic Integrity Violation

Anyone may use this form to report an alleged violation of the Policy on Academic Integrity. Faculty are expected to report students they have found responsible for violating the Policy on Academic Integrity to the Office of Student Conduct. The Office of Student Conduct maintains records of these reports as part of a student's conduct record.

File a Sexual and Gender-Based Misconduct Report

Report a concern about anything you think might be related to the CMU Sexual and Gender-Based Misconduct Policy (i.e. dating violence, domestic/intimate partner violence, sexual assault, sexual exploitation, sexual harassment, or stalking).
