Request to Waive Training Requirement
Central Michigan University recognizes that some training participants may have experienced sexual misconduct (dating violence, domestic/interpersonal violence, sexual assault, sexual exploitation, sexual harassment and/or stalking) prior to engaging in this training program. A participant that believes the training could bring up personal memories and strong emotions may opt out of taking this training.
By filling out the form below, you may waive the training requirement. The form below does not request specific information about why you are opting out of this training. However, the university recognizes that most individuals opting out of this training may desire additional information about resources available to them on campus and in the community. The university's Title IX coordinator will contact you via email shortly after submitting this form to provide you with this information and confirm receipt of your request.
In order to waive the training requirement, please fill in your full name, global ID (e.g. the part of your email address before "@cmich.edu"), and check the box below before hitting submit. After you submit this form, you will receive a confirmation message.
NOTE: This form alone does not provide enough information for the university to investigate a concern and should not be used to report an allegation of sexual misconduct. If you wish to report an allegation of sexual misconduct or discuss your options under the university's policies, please contact the university's Title IX coordinator. Information about filing a complaint regarding sexual misconduct with the university is also contained in the information you will receive from the Title IX coordinator.
