These reports help us connect individuals with resources, address issues and uphold our community standards. Please provide as much detail as possible, including names, titles and relevant information.

Report a concern
Discrimination, Harassment, and Sexual and Gender-Based Misconduct Reporting Form
Incidents of discrimination, harassment, and sexual and gender-based misconduct, which includes: dating violence, domestic violence, gender-based harassment, retaliation, sexual misconduct, sexual assault sexual exploitation, sexual harassment, and stalking.
Praise or Concern form
Students, faculty, staff, and the community can anonymously report praises or concerns.
Student Conduct
CMU Faculty, Staff, Students, and the public should use this link to report any possible violation of The Code of Students Rights, Responsibilities, and Disciplinary Procedures (The Code of Conduct) i.e. Academic Integrity, Classroom disruptions, Alcohol, Drugs, Fights, Threats, Hazing etc.
Care Report for a Student
Students at CMU and across the nation face many difficult and personally challenging situations throughout their college career. Mental health concerns, substance abuse, sexual assault, food and housing insecurity, and academic concerns are just a few of the challenges that students may deal with during their time as a student. CMU CARES is a comprehensive support service to help TAKE CARE of our students, faculty, and staff. CMU CARES provides students with helpful information and support navigating campus and community resources to support student well-being and success.

Students, Faculty, & Staff are able to submit a technology-related question, concern, suggestion, etc.
Legal name changes or corrections
Use this form to request a change to your legal name with the Registrar's Office. An email notification of this change will be sent to current faculty and staff.
Lived/Chosen name changes
Use this form for changing or adding a Lived/Chosen Name with the Registrar’s Office. An email notification of this change will be sent to current faculty and staff.
Student Activities and Engagement tracking
Students are encouraged, and at times required to maintain tracking of activities and engagement. If your scholarship, RSO, or course work requires you to be present at student events and programming, please fill out the form below.
Student Accommodations
Students who have a diagnosis could qualify for accommodations via Student Disability Services. This applies to students with both permanent and temporary disabilities. Once the form and documentation is submitted, Student Disability Services will reach out to arrange a meeting to discuss potential accommodations.
RAVE Guardian Campus Safety App
The RAVE Guardian Campus Safety App enables students, faculty, and staff to have a sense of security on campus. This app allows contact with the CMU police department and the Office of Civil Rights and Institutional Equity, as well as information for different resources. Users are able to share locations, set timers, and reach out for help when a phone call is not an option.
Work Order Request
The Work Order Request form is when individuals submit maintenance and non maintenance request to the Facilities Management team.
Food Pantry Registration Form
The CMU Student Food Pantry is here to support students who may be facing some form of food insecurity. Students who do not have access to food are strongly encouraged to submit a registration form. The Food Pantry also carries some hygiene, kitchen, cleaning, and school supplies. A visit is free of cost to students.
Work Order System
Students who live on campus in one of our residential communities may have maintenance or repair needs in their living unit. To best get needs met, the Work Order System allows you to submit a request and see the status of previous requests in one place.
