VSP (Vision Service Plan)
Staff, regular faculty, medical faculty and fixed term faculty in benefits-eligible positions can elect vision coverage. Additionally, eligible dependents may be covered. Employees pay the full premium.
Vision care benefits are provided by Vision Service Plan (VSP) using their Choice network of providers. VSP vision care benefits help you get the glasses or contacts you need to correct your vision. The program covers services from both VSP network and non-network providers. In addition to glasses and contacts, the plan offers discounts on laser vision correction. Treatment of a medical condition affecting your eyes, such as glaucoma or pink eye, is handled through your medical coverage.
Benefits when you use a VSP provider
When services are provided by a VSP provider, you pay only your copay(s) and the cost of any extras you want. Tints, special treatments, and specialized frames may be ordered; the additional charge you pay for these items will be at the discounted cost available to VSP participants. Benefits when you use a non-VSP provider
When using a non-VSP provider, you pay the provider's full charge and then submit a claim form and itemized receipt to VSP. You will be reimbursed for the applicable benefits. For details, please review the Vision Plan Comparison.
You can use the Out of Network Claim Form below or file your claim through the VSP member portal. Claims must be submitted to VSP within 12 months after the date the services were completed.
